I request up to 8 COVID-19 home test kit (“Kits”) per 30-day period or calendar month from Starcare Lab Solutions or its affiliated lab, including requests made on my behalf and/or dependents covered under my insurance plan (collectively, “Eligible Person”).
I understand that Starcare Lab Solutions or its affiliated lab will fulfill my request for the Kits if my insurance plan provides coverage. Information on testing resources for uninsured individuals can be found at https://www.hhs.gov/coronavirus/covid-19-care-uninsured-individuals/index.html.
I have the legal authority to make requests for the Kits on behalf of the Eligible Person.
By providing my phone number, I authorize Starcare Lab Solutions, its affiliates, subsidiaries, and subcontractors (collectively, “Vendor” or “Vendors”) to contact me via phone or SMS to confirm information for processing the Kit requests.
I authorize Starcare Lab Solutions and Vendors to use the Eligible Person’s Protected Health Information (“PHI”) for submitting insurance claims and as permitted under HIPAA.
I may cancel my request at any time by sending an email to info@mwclinical.com. However, this only applies to pending requests that have not yet been fulfilled.
StarCare Lab Solutions staff may pick up the Kits and arrange shipment on behalf of the Eligible Person.
I will not use the Kits for employment or resale purposes or to fulfill travel restrictions imposed by authorities.
The Kits should not be used for children under 2 years of age.
By agreeing to these terms, I acknowledge that no claim or dispute between the Eligible Person and Starcare Lab Solutions and its affiliated lab may be pursued or resolved as a Class Action. All claims or disputes must be resolved on an individual basis.
